🟠 Statement 1: Refund of NRST <br>I (fill in your name) am applying for a refund of the NRST I paid when I purchased the property at (fill in the specific address of the property you own).

🟠 Statement 2: Proof of Residence <br>I have occupied this property as my primary residence since (year/month/day) and have lived there until (now or year/month/day). Please also attach proof of residence for each month since you moved in (such as monthly bank statements, internet bills, phone bills, etc. Note: Home insurance, water, electricity, gas bills, and land tax bills are not recognized by the government)
I started to occupy the home as my principal residence on (YYYY/MM/DD) and have continued to occupy it to (Present or YYYY/MM/DD). Attached hereto as an exhibit is evidence for each month of my occupation of the home as my principal residence.

🟠 Statement 3: The actual beneficiary of the land <br>I am the actual beneficiary of the land, as of (date of house handover), i.e. Registration of Instrument No. (AANNNNNN) and Land Transfer Tax ($NNNNNN) and Foreign Buyer Tax ($NNNNNN) from the payment date.
I am the beneficial owner of the land and always have been since (set out date) when the beneficial ownership was acquired by the registration of Instrument No. (AANNNNNN) and the applicable land transfer tax ($NNNNNN) and NRST ($NNNNNN) was paid.

🟠 Statement 6: The beneficiary of all funds used to purchase the land and the dwelling thereon is (property owner, name), and the source of funds is (specify name). (If any part of If the source of funds is anyone other than the beneficial owner, such as a gift from a parent, please fully explain the circumstances).
All monies and other considerations utilized in the acquisition of the land relating to the conveyance were beneficially owned by (set out the name) and the source of the funds was (set out the name). Also attached is my bank account statement Bills showing I paid my mortgage, utility bills, property taxes, condo fees, and insurance.
That my annual income is $NNNNNN; and attached hereto as an exhibit are my Income Tax Return(s) and accompanying Notice of Assessment or Notice of Reassessment(s) since the year before the purchase of the land. Attached hereto as an exhibit are my bank account statements evidencing payment of the mortgage, utilities, municipal taxes, condo fees, insurance.

🟠 Statement 8: Spouse’s Annual Income If the applicant is purchasing land with his or her spouse:
My spouse's annual income is $nnnnnn; attached is my spouse's income tax return and a notice of assessment or notice of reassessment from the year before the land was purchased. Attached is my spouse's bank account statement proving he/she pays the mortgage, utility bills, property taxes, condo fees and insurance.
If the applicant bought the land with his or her spouse: That my spouse's annual income is $NNNNNNN; and attached hereto as an exhibit are my spouse's Income Tax Return(s) and accompanying Notice of Assessment or Notice of Reassessment(s) since the year before the purchase of the land. Attached hereto as an exhibit are my spouse's bank account statements evidencing payment of mortgage, utilities, municipal taxes, condo fees, insurance.

🟠 Statement 9: Funds for land costs If the applicant's income (and his or her spouse's income) is not enough to pay for the costs of purchasing a home (such as mortgage, utility bills, property taxes, condominium management fees, insurance and maintenance costs), please explain where the funds used to pay for the land costs came from.
If the applicant's income (and his or her spouse's income) is not sufficient to cover expenses for the house (eg, mortgage, utilities, municipal taxes, condo fees, insurance, and upkeep) explain where the funds to pay for the land and its expenses came from.

🟠 Representation 10: All Obligations All obligations, including any mortgage obligations, liabilities, acts or omissions, during my ownership of the property belong to me and will be performed by me.
All obligations, including any mortgage obligations, responsibilities, acts or omissions pertaining to the land during the period of time it was and is vested in me has and will be performed by me.

🟠 Statement 11: Sale and Purchase Agreement A copy of the Sale and Purchase Agreement is attached hereto as an attachment, pursuant to which I acquired beneficial title to the land and paid the NRST;
Attached hereto as an exhibit is a photocopy of the agreement of purchase and sale pursuant to which I obtained beneficial ownership of the land and paid the NRST;

🟠 Statement 12: Land Sale If the land is to be sold, all proceeds from the sale will belong to (specify name, usually the property owner)
If the land were to be sold, all proceeds of the sale will belong to (set out name)

🟠 Statement 13: Land Ownership If I die, the land will pass through my estate and be dealt with through applicable succession laws. Or, if not, explain how the ownership of the land will change after your death.

🟠 Statement 14: Right to enjoy the property I am entitled to all relevant benefits and income of the property, or the benefits and income of the property belong to (fill in the name).
